Private vs NHS Physio: Which Is Right For You?
If you're dealing with pain or an injury, one of the first questions is often whether to wait for NHS physiotherapy or pay for private treatment. There's no single right answer. It depends on your situation, your symptoms, and what matters most to you right now.
We're well placed to give you an honest answer. Jennifer spent four years working within the NHS spinal team at the Royal Devon and Exeter Hospital, and several of our team, Carmen, Daniel, and Amy, currently work in the NHS alongside their work here. We understand both systems from the inside, and we're happy to support you whichever route you choose.
The honest comparison
Here's how private physiotherapy at Quay Kinetics Physio compares with typical NHS MSK physiotherapy in Devon.
| Private (Quay Kinetics Physio) | NHS MSK Physiotherapy | |
|---|---|---|
| Cost | From £55 per session | Free at the point of use |
| Wait time | Same-week appointments | Varies by area; often several weeks to a few months for routine referrals |
| GP referral needed | No | Often not required; many areas now offer direct access via First Contact Physiotherapists, but this varies by GP practice |
| Session length | Typically 30-45 minutes | Often shorter, particularly for follow-up appointments, due to higher caseloads |
| Continuity of care | You see the same physiotherapist throughout your treatment | Can vary; you may see different clinicians across appointments depending on availability |
| Number of sessions | Flexible, based on your progress and goals | Often limited to a fixed number of sessions per course of treatment |
| Sport-specific expertise | Team includes MSc-level sports injury and sports medicine specialists, with experience treating climbers, runners, and triathletes up to elite level | Varies by service; not always available as a specific pathway |
| Setting | Two clinics, one inside a climbing centre with on-site assessment options | Hospital or community clinic settings |
When the NHS is the right choice
For many people, NHS physiotherapy is entirely appropriate and there's no need to pay privately. If your symptoms are mild, not significantly affecting your daily life, and you're able to manage with some patience, NHS physiotherapy provides good quality care at no cost.
The NHS is also the right route if cost is a genuine barrier. Physiotherapy is a core NHS service for good reason, and NHS physiotherapists, including several members of our own team in their NHS roles, provide thorough, evidence-based care.
If you have a condition that may need imaging, injections, or surgical input, NHS pathways are often the most direct route to specialist services, particularly where there's an established referral pathway from physiotherapy into orthopaedics or spinal services.
When private physiotherapy makes sense
Private physiotherapy tends to make the most sense in a few common situations.
You can't wait. If you're in significant pain, an important event or activity is approaching, or your symptoms are getting worse while you wait, same-week access can make a real difference to how quickly you recover.
You want continuity. Seeing the same physiotherapist throughout your treatment means less time spent re-explaining your history and a more joined-up approach to your rehabilitation.
You have a specific sporting goal. If you're training for an event, returning to climbing, running, or another sport, and want a plan built around that goal specifically, private physiotherapy allows for that level of focus.
You've already had NHS treatment and it hasn't resolved things. Sometimes a second opinion or a different approach is what's needed, and private physiotherapy can pick up where NHS treatment left off.
What if I'm not sure?
If you're unsure which route is right for you, you're welcome to book a private appointment for an initial assessment regardless. Many people use a private session to get a clear diagnosis and understanding of their condition, then decide from there whether to continue privately or return to NHS care with a clearer picture of what they're dealing with.
There's no obligation to commit to ongoing private treatment, and we're genuinely happy to point you toward NHS resources if that's the better fit for your situation.
Whichever route is right for you, we're happy to help you think it through.

No. You can book directly with us without a GP referral. If you're claiming through health insurance, check with your insurer first, as some policies require a GP referral or pre-authorisation before they'll cover treatment.

Not necessarily better, but different. NHS physiotherapists provide thorough, evidence-based care, and several of our own team work within the NHS alongside their work here. The main differences are practical: waiting times, session length, continuity of care, and how many sessions you can have. Whether private is "better" for you depends on which of those factors matter most in your situation.

Our sessions start from £55. For a full breakdown of pricing for physiotherapy, acupuncture, and sports massage, see our FAQs & Pricing page.

Yes. Many people start on the NHS and switch to private treatment if they want faster progress, more session time, or continuity with one clinician. We can pick up from where NHS treatment left off, working from any information you're able to share about your diagnosis and treatment so far.
-
No. Seeing a private physiotherapist does not remove you from an NHS waiting list or affect your NHS care. Many people use private physiotherapy to get started on treatment while waiting for an NHS appointment, then continue with whichever care best suits them once the NHS appointment comes through.

If we believe imaging, injections, or surgical input would help, we'll explain why and discuss the options. This may mean a referral back to your GP for NHS imaging or onward referral, or in some cases a referral to a private consultant. We'll help you understand the most appropriate route for your situation.
